I found all of these issues in the same 1985 home.   Obvious use of 1 drill bit per circuit by the electrical contractor.   This duct was disconnected at the bathroom exhaust fan.  Also disconnected at the other end.   The garage wall was never taped with mud.  Now 25 years later, multiple holes in addition to the joints.   The insulation spill results in an insulated ceiling in the garage.
